INFORMATION CLAUSE In accordance with art. 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data - General Data Protection Regulation (Journal of Laws EU L 119/1 of 4 May 2016 - "GDPR") the Warsaw University of Technology informs that: 1. The administrator of your personal data is the Warsaw University of Technology with headquarters at 1 Pl. Politechniki, 00-661 Warsaw. 2. The administrator has appointed a Data Protection Officer supervising the correctness of personal data processing, who can be contacted at the following email address: iod@pw.edu.pl. 3. Your personal data will be processed in order to conduct the survey "Monitoring the Careers of WUT Alumni" - the legal basis for processing your personal data being Art. 6 clause 1 point a GDPR; 4. The administrator will be processing personal data regarding: name, surname, e-mail address, student record number. 5. The Warsaw University of Technology does not intend to transfer your data outside the European Economic Area. 6. You have the right to access your personal data and the right to rectify it, the right to request deletion, restrict processing, the right to transfer data, the right to object to data processing, the right to withdraw consent at any time without giving a reason, without affecting the lawfulness of the processing that was carried out on the basis of consent before its withdrawal. 7. Your personal data will not be disclosed to other entities (administrators), except for entities authorized under the law. 8. Access to your personal data may be granted to entities (processors) to which the Warsaw University of Technology outsources the performance of activities involving the processing of personal data. 9. Providing your personal data is voluntary, but failure to do so will prevent you from the possibility to take part in the "Monitoring the Careers of WUT Alumni" survey. 10. Your personal data will be processed until the consent is withdrawn, and for the period required by law or to secure any claims. 11. The Warsaw University of Technology does not use automated decision-making in relation to you, including it does not perform profiling of you. 12. You have the right to lodge a complaint with the supervisory body - the President of the Office for Personal Data Protection, if you feel that the processing of your personal data violates the provisions of the GDPR.
